Cardiac Screenings

St. Francis Hospital & Heart Center® (Roslyn, NY) conducts free cardiac screenings for student athletes, grades 9-12, to help diagnose potentially life-threatening cardiac conditions. An electrocardiogram (EKG) and echocardiogram will be performed and results are available immediately.

Requirements

Please call 516-629-2013 for registration, forms and any questions before your appointment.

  • Students must bring picture identification and consent and medical history forms completed by a parent or legal guardian.
  • Pre-registration prior to appointment.
  • Students must be accompanied by a parent or legal guardian.
