Medicaid Service Coordination is an initiative of the New York State Office for People with Developmental Disabilities Office (OPWDD). It is targeted to offer services to individuals with a diagnosis of a developmental disability.

The mission of Medicaid Service Coordination is to help individuals with developmental disabilities get access to the services and support they need to enhance and maintain a level of functioning where they have personal satisfaction and live in the environment of their choice.

The following values are inherent in this mission:

  • All individuals with developmental disabilities have the right to full participation in the community in which they live.
  • All individuals with developmental disabilities have a right to timely and appropriate health and mental health rehabilitation and social support services to maintain their well-being in the community.
  • All individuals with developmental disabilities should be able to identify their own valued outcomes and be supported in attaining these outcomes.

Utilizing a person-centered philosophy and approach, services are determined by the circumstances, needs and desires of each individual. Together with the individual, and the advocate of their choice, the service coordinator assists in the development and coordination of services and support. These individualized services empower the person toward independence, inclusion and productivity.

The Individualized Service Plan (ISP) is a guide for self-determined outcomes. The ISP is reviewed at regular intervals and incorporates input from team members who will assist the individual to improve the quality of their life. Coordination of services is an ongoing process. Contact with the individual at home, at day activities or in the community are part of this process. This contact, along with input received by ancillary contacts, allows the individual to discuss their ongoing progress with the service coordinator and allows the service coordinator to address any personal or safety issue needs.

Medicaid Service Coordination serves individuals who have a documented diagnosis of a developmental disability. Individuals may refer themselves by contacting Maryhaven Center of Hope directly or by having a vocational counselor, school counselor or therapist contact Maryhaven directly.

Proof of a developmental disability must be provided in the form of a current psychological evaluation and current psychosocial evaluation along with a current physical. Individuals will be assisted to obtain and maintain financial entitlements such as Medicaid. Medicaid Service Coordination is funded by Suffolk County Medicaid. Individual financial circumstances are discussed privately.

Wisdom Gardens

Nestled in Port Jefferson Station, Wisdom Gardens was created by Maryhaven Center of Hope as an affordable housing opportunity for seniors. The site is conveniently located near shopping, medical facilities, recreation, libraries and transportation. Its New England style buildings face green courtyards and recreation areas.

Wisdom Gardens consists of 40 apartments that are handicap accessible and feature kitchens with stove, refrigerator and wood cabinetry. Amenities include a community center with full kitchen, recreation room and library, social services offices and laundry, an onsite social services coordinator, and a Maryhaven-run transportation service to bring residents to local shopping and recreational centers. Part of what makes Wisdom Gardens unique are the six apartments reserved for individuals with developmental disabilities.


Maryhaven Center of Hope operates its own transportation system with a fleet of 235 vehicles. Our transportation center has a full-service maintenance garage and refueling station. In addition, we provide transportation for therapy services to Catholic Health's St. Charles Hospital in Port Jefferson, NY.
