Elizabeth (Liz) Potter, RN, the 2019 Manager of the Year for Catholic Health Services’ (CHS) Catholic Home Care (CHC) is driven by an intense desire to help people in need.

 

 

A resident of Smithtown, Potter has been an integral member of CHS’s Home Care team for the past 14 years. In 2006, she joined CHS’s Good Samaritan Hospital Medical Center as a home care nurse. Four years later, Potter moved to CHC where she is a nurse manager for Team 7.

 

 

Her 40-person team of registered and licensed practical nurses, social workers and physical, occupational and speech therapists stretches from St. James to Port Jefferson in Suffolk County.

 

 

“Receiving this honor from Catholic Home Care means a great deal,” said Potter. “But this award is not only about me. It’s also about the hard work and dedication of my team in the field who are caring for patients daily.”

 

 

She earned the title of Manager of the Year with high quality scores for her team, including patient satisfaction, communication with patients and overall patient care. The team’s scores were the highest within CHC. Also, she was lauded for mentoring new managers and her willingness to volunteer and serve local residents as part of CHS’s Healthy Sundays program.

 

 

“Liz’s leadership and experience is an asset for her team and the patients they serve,” said Catholic Home Care Director of Patient Services Mary Frawley. “She continually reviews our procedures to see how we can further to raise the bar on the home care services we offer.”

 

 

Daily, Potter is responsible for developing a plan of action for her team to help patients who have been discharged from a hospital or nursing home recover quickly. Also, she communicates with patients and family members to discuss the services CHC offers to assist in developing individualized plans for patients.

 

 

Beyond the administrative work, Potter also spends time in the community providing care to patients in their homes.

 

 

“I like being out there,” she said. “It gives me a chance to work directly with our patients, speak with them and get input on our services. This feedback is vital for us as we continually strive to provide the highest level of service to people in the comfort of their homes.”
