Catholic Health MyChart makes it easy for you to save time at your doctor's appointment. With eCheck-in you can fill out required forms and keep your information up-to-date, including:

  • Address & contact information
  • Current health concerns
  • Medications
  • Allergies
  • Insurance
  • Past medical history
  • Pharmacies

How To Use eCheck-In

Step-by-step instructions:

  1. Log in to your Catholic Health MyChart account online or via your Apple Catholic Health app or Android Catholic Health app.
     

  2. Go to 'Appointments & Visits' and find your upcoming appointment.
     

  3. Click the green 'eCheck-In' button next to your appointment.
     

  4. Fill in each page with the appropriate information. Click 'Next.'

     

  5. Click 'Finish' to submit after you have completed the final page.

Existing MyChart users: Sign in with your username and password.

New MyChart users: Enter your authorization code. Don't have one? Call 631-465-6100 for assistance.
